Please download all camp forms and mail to our Administrative office or turn in your forms at your local San Francisco Clubhouse. Please be sure to fully complete all forms. Any uncompleted forms will be returned and may jeopardize your child's opportunity to attend camp.

Operation Purple® Camp
Session 1: June 24 - July 3 (Ages 10-14)
Session 1A: June 24 - June 30 (Ages 8-9)
If you are attending Operation Purple® Camp please complete the below forms. However you must also register with the National Military Family Association (NMFA) first. Operation Purple® Website http://www.nmfa.org/site/PageServer?pagename=op_default this will then bring you back to our Website and then you can complete the below OPC forms
Dear parent only complete these forms if your child/children are attending Operation Purple® Camp at Camp Mendocino. If your child/children are attending any other sessions at Camp Mendocino they need to complete the above Camp Mendocino forms.
